112 Annual Fee Letter PLEASE READ CAREFULLY
In order to maintain your mining operation in compliance with Colorado laws , the
annual fee, report, and map required by C.R.S. 34-32-116(3 and 4) and
34-32-127(2)(a)(II)(A-D) must be submitted annually on or before your permit
anniversary date. The annual fee (this is NOT a renewal fee) , report, and map are
for the 1991-92 mining and reclamation season, and are due and payable, unless your
reclamation bond has been released in its entirety, by the anniversary date listed
below.

DIVISION ON OR BEFORE YOUR ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A
BOARD ORDER TO PAY CIVIL PENALTIES EQUAL TO YOUR ANNUAL FEE AMOUNT OF $430.00 WILL
AUTOMATICALLY BE ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL BE GRANTED
TO ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE
(C.R.S. 34-32-120) UNTIL THE VIOLATION IS ABATED.
If your records indicate different facts than those stated above or if you have
already forwarded your remittance , report, and map to us please inform this office
as soon as possible. Any questions concerning your annual report or fees , may be
directed to Barbara Chiappone at the Division office.
Please return the enclosed form with your completed Mao , and a check or
money order for your annual fee, payable to the Mined Land Reclamation Division ,
