As a result of freezing, the pipe failed or split at three <br />locations between 91-1 V and 93-3 V. W,~tmter temperatures on 12/26/98 allowed flow from barren liquor line to leak through 2" <br />pipe to split sections of pipe and flow on the ground and down We hill approximately 600' to near the natural gas substation. <br />Approximately one fool of snow slowed down the rate of overland flow and spill did not reach the dry strearn bed east of natural <br />gas substation. The 2" valve for pipe corning from the 8" barren liquor line was defective or inoperable from build up of bicarb <br />scale. Therefore, [he two inch pipeline h:~s been pressurized for several years, but did not fail until 12/98 cold temperatures caused <br />damage to pipeline. Loss of barren liquor Link volumes had been noticed shortly prior to notification of the leak. Based on tank <br />levels and barren liquor line pressure, leak was for approximately 140 minutes. After flow was shut off a 2" plug was installed in <br />valve to eliminate flow to 2" pipeline. Prior to use of the 2" pipeline a new valve will have to be installed. <br />CAUSE AND EXTENT OF PERSONNEL [NJURY: None <br />OTHER FEDERAL, STATE AND LOC/J. GOVERNMENT AGENCIES NOTIFIED: llMG <br />ACTION TAKEN TO PREVENT RECURRENCE: As inspection of all 2" wash water and barren liquor lines off the main lines <br />will be made [o verify valves are turned ofl'. [n the future inoperable values will be replaced immediately if stuck in open position. <br />Operators will be reminded to watch for I:.~dcs during daily welUpipeline inspections, and if flows, line pressures or tank levels <br />change unexpectedly, they should suspect a leak in well field piping.
