Lung <br />Division Dnrc m. <br />Annual fees are not permit renewal fees, they are fees for having a valid <br />reclamation permit, and must be paid each year until reclamation responsibility <br />release is granted. If you have requested reclamation release from the Division <br />but your permit is not terminated by your anniversary date listed below the annual <br />fee must be paid. If the Division does release this permit before the anniversary <br />date please do not pay this fee. <br />The fee is for LAST YEARS mining and reclamation season, and MUST be paid even if <br />the mine was in an inactive state. It is due and payable ON OR BEFORE the permit <br />anniversary date shown below. <br />PLEASE NOTE: IF THE ANNUAL FEE, ANNUAL REPORT, AND MAP ARE NOT RECEIVED ON OR <br />BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A BOARD ORDER TO P, <br />CIVIL PENALTIES IN THE AMOUNT EQUAL TO THE ANNUAL FEE OF $550.00 WILL BE <br />AUTOMATICALLY ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL 8E GRANTED TO <br />ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE <br />(C.R.S. 34-32-120> UNTIL THE VIOLATION IS ABATED. <br />Division records indicate you owe $550.00. <br />Permit No.: M-77-320 <br />Operation Name: 8-Mile Clay Mine` <br />Anniversary Date: 06/29/94 <br />Annual Fees for the Year(s): 1994 <br />Total: $550.00 (Due on your Anniversary Date) <br />If your records indicate different facts than those stated above please notify <br />Barbara Chiappone at the Division office. <br />Please return the enclosed annual report FORM, a MAP, and CHECK or MONEY ORDER made <br />payable to the Division of Minerals and Geology, 1313 Sherman Street, Room 215, <br />Denver, Colorado 80203.
