Lung <br />Di~isian Dmrclur <br />110(2) ANNUAL fEE INVOICE and REPORT REQUEST <br />PLEASE READ CAREFULLY <br />ANNUAL FEES ARE NOT PERMIT RENEWAL FEES, they are fees for having a reclamation <br />permit and are a requirement of Colorado Reclamation Laws. You must pay them each <br />year until reclamation responsibility release is granted. The fee is for LAST <br />YEARS mining and reclamation season, and MUST be paid even if the mine was in an <br />• inactive state. It is due and payable ON OR BEFORE the permit anniversary date <br />PLEASE NOTE: IF THE ANNUAL FEE, ANNUAL REPORT, AND MAP ARE NOT RECEIVED ON OR <br />BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION AND A BOARD ORDER TO PAY <br />CIVIL PENALTIES IN THE AMOUNT EQUAL TO THE ANNUAL FEE OF $225.00 WILL BE <br />AUTOMATICALLY ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL BE GRANTED TO <br />ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE <br />(C.R.S. 34-32-120> UNTIL THE VIOLATION IS ABATED. <br />shown below. <br />If you have requested reclamation release from the Division but your permit is not <br />terminated by your anniversary date listed below the annual fee MUST be paid.
