Please be advised that according to records of the Division of Minerals and Geology the
following annual fee(s) and report(s) are past due.
The Division MUST RECEIVE ALL FEES DUE NO LATER THAN 9 P.M. ON March 19, 1997 (IF THIS
DATE FALLS ON A WEEKEND OR STATE RECOGNIZED HOLIDAY YOUR ANNUAL FEE WILL BE DUE ON THE
NEXT REGULAR BUSINESS DAY), or a Notice of Violation, Cease and Desist Order from ALL
mining related activities, Civil Penalties equal to the amount of the annual fee(s)
due, and a Notice of Formal Public Hearing before the Mined Land Reclamation Board for
consideration of possible Hermit revocation and financial warranty forfeiture will be
issued for each account.

operator who is currently found to be in violation in this state (C.R.S. 34-32-120).
