On March 20 1984, the Division of reined Land rtecla~nation
received an annual fee payment in the arnount of $ 350.00 To this date,
we have no record of haviny received the annual report as requested.

Nrticle 3l of Title 34, C.R.S. 1973, as amended and by the Rules and
Reyulations promulgated tnereunder. Your attention is directed to the
following provisions from Rule z.4 for operators noldiny a reyular, 112
pernit and Kole 3.4 for operators Holding a limited impact, 110 permit.
2.4 ,annual Reports
2.a1 On the anniversary date of each permit year, the operator snall
suo~nit a report and map (to a scale of Hoc less tnan l0U' to the
incn and not more than tibU' co the incn), indicating the phases
of the reclamation plan wnicn have been completed, correlated
witn the timetable. Tne report and map shall contain the
following information:
(1) Location and numoer of acres affected during that year,
(~) Locations and numoer of acres reclaimed for that year,
(.i) The location and numoer of acres in various stages or
reclamation (oackfilled, graded, topsoil replaced,
revegetated),
(4) Tne species of vegetation planted, the locations and the
approximate dates of planting,
(5) The type and approximate quantity of fertilizers, organic
material or soil conditioners used and location of use,
(o) .an appraisal of the success of reclamation efforts, and in
the event of failure of any efforts, a short narrative on
the suspected or detennined causes of sucn railure,
(7) estimates of the location and nwnber of acres to oe
arfected in the coming year, and
i3) Tne effective date of t:~e information provided aoove snail
