Extent of current disturbances. <br />There were no disturbances in 2005. (see attached map from the 2005 annual report). <br />2. Reclamation accomplished to date and during preceding year. <br />The reclaimed end use for the area under the mining permit is water storage. The reclamation <br />accomplished to date revolved around the construction of the slurry wall (completed in 2002) and inlet <br />and outlet facilities needed to operate the storage facility (completed in 2005). In 2003, Greeley's 25a' <br />Ave. gravel lake storage project met the Colorado State Engineer's Office performance standazd for lined <br />gravel pits. After meeting this standard for lined reservoirs, Greeley initiated and substantially completed <br />the inlet and outlet facilities for the storage facility irr 2004. Greeley began removal of tamarisk. from the <br />slide slopes in 2005. <br />3. New disturbances anticipated to occur during upcoming year. <br />LaFazge completed mining the berm between the lakes in 2004 and opted to discontinue removing <br />material below the present bottom of the existing lake(s). The disturbed slopes were reclaimed per the <br />Reclamation Plan. There aze no planned disturbances for 2006. <br />4. Reclamation to be performed during upcoming year. <br />The eastern portion of the site will be graded and debris will be removed in 2006. <br />5. Dates for the beginning of active operations. <br />Active mining began at the site in December of 2003. <br />6. Date active operation ceased for the year, if any. <br />December 2004. <br />
