Lnns <br />IvA*Jl Drvis ion Director <br />RE: Division's Review of 1994 AHR, Hayden Gulch Mine, Permit No. <br />C-80-003, Hayden Gulch Terminal <br />The Division reviewed the 1994 Annual Reclamation Report and Annual <br />Hydrology Report for the Hayden Gulch Mine. The Division sent a <br />letter, dated November 30, 1995,. to TerraMatrix which contained <br />fourteen questions concerning the report. This memo to file has two <br />main parts. First is a summary of the Division's review of the 1994 <br />report, labeled section 1 through section 4. Second is the list of <br />fourteen questions sent to Terramatrix, labeled section 5. <br />1. PERMIT MONITORING PLAN VS AHR MONITORING PLAN <br />The 1994 ARR showed surface water and groundwater monitoring site <br />locations on Map 1, Environmental Monitoring and Sampling Sites. <br />The permit application shows the surface water monitoring locations <br />on the map entitled "Surface Water Sampling Site Map", found in <br />Volume 1B. Both maps are in agreement as to the location of the <br />upstream surface monitoring site, labeled UHG-1 in the 1994 ARR. <br />However, the permit application map shows the downstream monitoring <br />site as being just below the outfall of pond HG-8. The 1994 ARR map <br />shows the downstream monitoring location, labeled LHG-1, as being <br />about 3500 feet further downstream. The Division raised this <br />possible discrepancy, in question no. 14, to TerraMatrix. <br />The groundwater monitoring sites are located on the permit <br />application map entitled "Final Reclamation Map, 1988." The well <br />locations coincide with the well locations shown on the 1994 ARR <br />map. The Division pointed out, in question no. 2, that there was a <br />discrepancy in well annotation for UC-2 on Map 1 of the 1994 ARR. <br />Well HG-1 monitors groundwater down dip of the shop area. Well HG-2 <br />monitors groundwater down dip of the mine area. Well UC-2 monitors <br />only water depth up dip from the shop area. <br />The locations of the springs and seeps and of the NPDES outfall are <br />shown on the 1994 ARR Map No. 1, Environmental Monitoring and <br />Sampling Sites. <br />
