Walker subject: Review for Compliance date: July 23, 2003 Rule 4.09.1(11) Inspection Rule 4.09.1(11)(a) The f Il shall be inspected for stability by a registered engineer or other qualified professional specialist experienced in construction of earth and rockfill embankments under the direction of a qualified professional engineer at least quarterly' throughout the construction and during the following critical construction periods2: (1)removal of all organic material and topsoil, (2)placement of underdrainage systems, (3) installation of surface drainage systems, (4) placement and compaction of fill materials, and (5) revegetation. The permittee is in compliance with this rule. 'This is a quarterly report. A registered engineer accomplished both the inspection and certification. z Installation of surface drainage systems was a critical construction period encountered during the time period of the report. Rule 4.09.1(11)(b) The qualified registered engineer shall provide to the Division a certified report within 2 weeks after each inspection that the fill has been constructed as specifted in the design approved by the Division. ZThe report shall include a description of any appearances of instability, structural weakness, and other hazardous conditions observed during the inspection. A copy of the report shall be retained at the mine site. The certification indicates that the fill was inspected on June 20, 2003. Thus the report was due July 10, 2003.
