However, the Division has not received a completed annual report which was originally <br />due December 23, 1998. Failure to submit an annual report may result in enforcement action with associated <br />civil penalties. <br />Your attention is called to the requirements of C.R.S. 34-32.5-116 (3) (a): <br />Each year, on the anniversary date of the permit, an operator shall submit the ant=ua[ fee specified <br />in section 34-32-127 (2), a report and map showing the extent of current disturbances to affected land, <br />reclamation accomplished to date and during the preceding year, new disaurbances that are <br />anticipated to occur during the upcoming year, reclamation that will be performed during tlfe coming <br />year, the dates for beginning of active operations, and the date active operations ceased for the year, <br />if any. <br />Please complete the enclosed annual report form, required by Rule 1.15, and return it to the Division within <br />30 days of the date of this letter. <br />Thank you for you cooperation with this matter.
