' FILE 80-121 MINE Pierce Pit ~II'I~II~II~I'~~ ~I~ sss DATE 5-31-84 116(1)(a) 2.41 Annual Reports ..,,,~ 2.41 On the anniversary date of each permit year, the operator shall submit a report and map (to a scale of not less than 100' to the inch and not more than 660' to the inch), indicating the phases of the reclamation plan which have been completed, correlated with the timetable. The report and map shall contain the following: (1) Location and number of acres affected during that year. One half acre (See mining/extraction mao 1 (2) Locations and number of acres reclaimed for that year. It is scheduled for a later date this vear approximately one half acre (See reclamation plan map) (3) The location and number of acres in the various stages of reclamation (backfilled, graded, topsoil replaced, reveg- etated). 2 acre along west bank going north is scheduled to be backfilled with topsoil and seeded (4) The species of vegetation planted, the locations and approximate dates of planting. Seed that will be planted, side oats grama, blue grama. We have it scheduled for later this year. It will be along west bank 2 acre. (5) The type and approximate quantity of fertilizers, organic material or soil conditioners used and location of use. According~ to permit fertilization and soil conditions will not be necessary. (6) An appraisal of the success of reclamation efforts, and in the event of failure of any effort , a short narrative on the suspected or determined caused of such failure. None at tihis time (7) Estimates of the location and number of acres to be affected in the coming year. roximately one half acre (8) The effective date of the information provided above shall be no more than 45 days prior to the anniversary date.
