fee, the enclosed Notice of Intent to Continue Mining Operations (your annual
report), and your map, as reouired by the statute, on or before your anniversary
date. If you cannot include these materials with t e elf e; o e se indicate on the
enclosed annual report form how soon we should expect to receive these materials.
To bring the Division up-to-date as to the reclamation on the site, please include
in your annual report, and mao the following information:
Your m~3p should be to a scale of not less than 100' to the inch and not more than
660' t~~ the inch, it should indicate the phases of the reclamation plan which have
been c~~mpleted and correlated with the timetable as reouired. Your report and map
shall r:ontain the following information:
1) Location and number of acres affected during that year;
21 locations and number of acres reclaimed for that year;
