In order to continue your mining operation, annual fees, report, ono map are
required pursuant to 34-32-11b(1)(a) of the Colorado Mined Land declamation
Act, C.R.S 1973, as amended. Fees are based upon a schedu]e of X350.00 if the
operation is an open ~nininy operation of 10 acres or mu re and $275.00 fur any
other operation lU acres or +nore.

Submittal of a report and map (to a scale of not less than l0U' to the inch
and not more than 6bU' to the inch), intlicating the pnases of the reclamation
plan which have been completed and correlated with the timetable is required.
The report and map shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and numuer of acres reclaimed for that year,
3) The location and number of acres in the various stages of
reclamation (backfilted, graded, topsoil replaced, revegetatea),
4) The species of vegetation planted, the locations and approximate
natas or planting,
