Telluride Gravel provides the following information to comply with the Annual Report requirements:
I. Status of Permit Boud
The required bond is underwritten by Liberty Mutual Insurance Company, effective 2-27-06.
See attached copy of bond.
2. Storm Water Management
Telluride Gravel currently complies with the State of Colorado Storm Water Management Plan
Requirements.
3. Weed Control
Telluride Gravel performs weed control to comply with permit requirements. Weeds are maintained by
San Miguel County through a paid agreement and weeds are pulled by Telluride Gravel and disposed of
off-site by trash removal services.
4. Map of Current Disturbances and Reclamation Activities
Mining activities are inactive. Changes in locations of stockpiles and shops/plants are shown on the
attached drawing(s). Reclaimed areas are also identified. These areas were confirmed during a recent
inspection by Mr. Bob Oswald, Colorado DMG Inspector, on 5-12-06.
