(Pape 21 <br />MINE ID # OR PROSPECTING ID # M-94-115 <br />INSPECTION DATE August 28. 1997 INSPECTOR'S INITIALS TDG <br />OBSERVATIONS <br />This was apre-operational, partial inspection of the proposed Orion Pit. <br />The applicant has not posted an identification /application notice sign at the entrance to the site. <br />During the inspection, a number of permanent man-made structures were observed on and within 200 feet of the proposed 368 <br />acre affected area boundary. These structures consist of such items as houses, buildings, barns, sheds, fences, irrigation ditches, <br />water impoundments, discharge and conveyance structures, water lines, power lines, telephone lines, and public & private <br />roadways. However, the maps provided in the application do not, as required by the Construction Materials Rules and Regulations, <br />show the location and type of all such structures. In addition, the application does not contain any damage compensation <br />agreements for all such structures, nor does the application contain an appropriate engineering evaluation demonstrating that such <br />structures shall not be damaged by proposed construction materials eMraction operations. <br />In regard to water information, during the inspection several stock water ponds, trout rearing ponds, springs, irrigation ditches, water <br />discharge structures, and water conveyance structures were observed within and/or hydrologically adjacent to the proposed affected <br />area.
