reclamation permit, and must be paid each year until reclamation esponsibility
release is granted. If you have requested reclamation release fr m the Division
but your permit is not terminated by your anniversary date listed below the annual
fee must be paid. If the Division does release this permit befor the anniversary
• date please do not pay this fee.
The fee is for LAST YEARS mining and reclamation season, and MUST be paid even if
the mine was in an inactive state. It is due and payable ON OR B FORE the permit
anniversary date shown below.
PLEASE NOTE: IF THE ANNUAL FEE, ANNUAL REPORT, AND MAP ARE NOT R CEIVED ON OR
BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION AND A OARD ORDER TO P
CIVIL PENALTIES IN THE AMOUNT EQUAL TO THE ANNUAL FEE OF $225.00 ILL BE
AUTOMATICALLY ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHA L BE GRANTED TO
ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS ST TE
