Main Street <br />PO Box 1412 <br />- ~ - Ouray, CO 81421 <br />TO: Division of Minerals and Geology <br />1313 Sherman Std, Room 215 <br />Denver, CO 80203 <br />RE: Permit No M-2001-054 <br />Operation Name: i Bridalveil <br />Anniversary Date: October 28, 2004 <br />Total Fee Due: $281 <br />Enclosed is our Annual Report for the yeaz, anniversary date October 28, 2004, with a <br />map showing work complete to date, and check in the amount of $281. <br />We have produced and sold 2,734 yards of 1 inch Road base from October, 2003 to <br />October, 2004. The channel and the settling pond's 3 to I slopes are 50% completed. We <br />are working with C-DOT on access requirements at State Highway 550. <br />We are in the process of getting'a revised plan, and have submitted alt paperwork for that. <br />This will entail a new access, for which a state access permit has been obtained, and a <br />larger level one area for screening material. <br />Level one is approximately 50%: complete. It is anticipated that we will begin excavation <br />on level 2 this yeaz. Three-to-one slopes and revegetation will be completed around the <br />settling pond and beside the roadway within the next few months, when material is <br />available from that excavation of bench 1. To date, reclamation is complete at the north <br />end of the property. i <br />~~ <br />
