Enclosed please find the annual report for [his operation, submitted on behalf of my cli nt, Transit Mix of
Pueblo. Included is the form, the map, an additional page, and a Cashiers's Check (NC 3 26339) for
$550.00 for part of the annual report fee of$688.00. Unfortunately, the $550.00 amount that was shown
on the form is in error due to a problem with the DMG database not updating the ann report form.
Mark Klune of Transit Mix of Pueblo has been notified of the deficiency and is sending in additional
check to make up the additional amount to bring the total fee paid to $688,00. That sho ild arrive soon
