W. Jack Clark
6052 S. Newport Street
Centennial CO 80111-4458
Phone/Facsimile 303/221-6588
RECEIVED
JAN 10 2003
January 10, 2003 Giuision of Minerals and Geology
Ms. Suzi Ericksen
Program Assistant
Colorado Division of Minerals and Geology
1313 Sherman St., Rm. 215
Denver CO 80203
RE: Annual Report for Permit Number M-1997-054
Dear Ms. Ericksen:
This letter transmits the Annual Report for the above Permit for the Parkdale quarry.
Please find enclosed the signed reporting form, annual fee, and map.
You will note on the map that Phase III of the gravel mine is planned for disturbance in
2003. This area is within the affected lands portion of the Permit, but a bond has not
been posted for its reclamation. A Technical Revision, with calculations for reclamation
costs, will be submitted to the Division in the near future requesting approval for this
expansion.
Due to changes in personnel at the quarry, water samples were inadvertently not collected
during the spring of 2002. Quarry personnel inspected the sampling sites in the fall of
2002, and the sites were found to be dry in this abnormally dry year. Water samples will
be collected and analyzed this spring where sufficient water is present for sampling.
If you have any questions, please contact me.
Sincerely,
~r~~JCI/"" -
W.
