Fees are based upon a schedule of <br />$350.00 if the operation is an open mining operation of ZO acres or more and <br />5275.00 for any other operation 10 acres or more. <br />Also, to bring the Division up to date as to the mining and reclammation on the <br />site, please submit a report and map (to a scale of not Less than 100' to the <br />inch and not more than 660' to the inch), indicating the phases of the reclama- <br />tion plan which have been completed, correlated with the timetable. The report <br />_ and map shalt contain the following information: <br />1) Location and number of acres affected during that year, <br />2) Locations and number of acres reclaimed for that year, <br />3) The location and number of acres in the various stages of reclamation <br />(backfilled, graded, topsoil replaced, re vegetated), <br />4) The species of vegetation planted, the locations and approximate dates <br />of planting, <br />5) The type and approximate quantity of fertilizers, organic material or <br />soil conditioners used and location of use, <br />6) An appraisal of the success of reclamation efforts, and in the event <br />of failure of any efforts, a short narrative on the suspected or determined <br />causes of such failure, <br />7) Estimates of the location and number of acres to be affected in the <br />coming year, and <br />8) The effective date of the information provided above shall be no more <br />than 45 days prior to the anniversaru date. <br /> <br />
