APPENDIX A
Quarterly Inspection Report
New Elk RDA
1. Seepage and minor slump of cover material observed above Bench 3 (refer
to photo page A-2). Also, a minor seep was observed at west end above
Bench 3 {Photos Fig. A-3). At this time, the seeps appear minor and we do
not believe large scale stability is affected. Will re-observe conditions in 3'"
Quarter Inspection. At that time, we anticipate recommending repair of seeps
by a trench excavation through the cover (2 feet wide and 2 feet into waste),
placement of filter fabric, and re-filling excavation with gravel to allow seeps
to drain. If conditions worsen, then investigation to evaluate water conditions
inside the pile above Bench 3 may be required.
2. Small seep out of sandstone above pile surface in NW corner (Photo Fig. A-4).
Seepage considered minor. Recommend re-grading of immediate area to
reduce ponding. If pile construction resumes, drain from this area connecting
to underdrain would be prudent.
PICKET WIRE PROCESSING, LLC
NEW ELK, RDA
CTLR 48,170 A-t
