Section 34-32-110(4) of the Colorado Mined Land RAFlamation Act requires that,
"... the Operator shall annually file, on the anniversary date of the
permit, a Notice of Intent to Continue Min ng Operations. The Notice
of Intent shall contain an annual fee of ~5 .00 and a map or statement
that shall indicate: (a) the land affected uring the previous year;
(b) the land to be affected during the cumin year; and (c) aqy land
