Enclosed with this cover letter summarizing site activities <br /> during the year ending September 29, 2001, are: 1) a signed DMG <br /> "ANNUAL FEE and REPORT REQUEST" form; 2) a check for$688.00 for <br /> the annual fee due on or before September 29, 2001; and 3) a report map <br /> depicting site mining and reclamation activities for the reporting and <br /> upcoming years. <br /> During the report year mining activity occurred as clean-up re-mining <br /> within Phase 3A and through development of Phase 5 as per Exhibit D — <br /> Mining Plan Map in the permit and amendments. Other site <br /> improvements included minor relocation of the plant wash water discharge <br /> ditch; creation of a new wash water settlement pond; initial construction of <br /> the berm between future water storage cells No. 1 and No. 2 (shown as <br /> reclaimed during the report year); and clean-up of considerable historic <br /> site debris and abandoned equipment. In addition, some of the <br /> overburden stockpile at the north end of the site was removed for <br /> construction of the E-4701U.S. 85 Interchange presently under <br /> construction. <br /> Mining activities at the site occurred within two general areas, which <br /> combined comprise approximately 24.7 acres that were re-disturbed or <br /> 2329 wen Main street newly disturbed. Reclamation for the berm comprised approximately 2.9 <br /> Suite 201 acres and consisted of compacted fill placement.
