The Notice <br />of Intent shall contain an annual fee of $50.00 and a map or statement <br />that shall indicate: fa) the land affected during previous year; <br />(b) the Land to be affected during the coming year; and (c) any Zand <br />reclaimed during the preceding year." <br />Based upon Our records, the anniversary date of your operation is March <br />and annual fees for the year(s) 82,83 , totalling $ 00.00 <br />- are due on that date. <br />If your records indicate different Facts, please inform this office as soon as <br />possible; otherwise, we wi1I expect to receive the annual fees as regtu red by <br />the statute on or before your anniversary date. Also, please include the <br />required Notice of Intent to Continue Mining Operations to bring the Division <br />up to date as to the progress of mining and reclamation on the site.
