Permit - <br /> No. M-83-033, Annual Report <br /> FEDERAL TAX ID# 84-0517799 <br /> Our files indicate that the anniversary date for this <br /> permit is June 23 . We have a NPDES permit and SWMP for the site as <br /> required by the State Health Department. <br /> This pit HAS NOT been granted a Temporary Cessation and it HAS <br /> operated more than 180 days this year. The Financial Warranty for <br /> this operation is $75, 000, the permitted area is 125. 88 ac. ± and <br /> the pit is divided into 7 Reclamation Phases. A sign that conforms <br /> with rule 6.7 Signs and Markers is at the entrance to the quarry on,, <br /> the scale house. Please refer to the Annual Report Map for the <br /> location of areas discussed in this report. This is a quarry <br /> operation, as a result we remove large amounts of material without <br /> disturbing large areas of land. <br /> We will deal specifically with each of the Annual Report <br /> requirements as outlined in Rule 1. 15 of the Rules and Regulations, <br /> and the annual notice letter. <br /> (1) No new land was disturbed this year, mining continued on the <br /> area previously stripped in Phase I. Total disturbance to <br /> date in Stage I is 31.07 acres ±. The floor area increased by <br /> 2 .76 acres ± as the face of the mine moved into the stripped <br /> area. The current face area is approximately 4. 53 acres ±. <br /> (2) No reclamation was required this year. Minor amounts of <br /> backfill continue to be placed in the valley on the eastern <br /> side of Phase I as approved. The table below outlines the <br /> various stages of reclamation at the pit. <br />
