First of all, no check for $50.00 was enclosed. Please send this check at your earliest convenience. Second, for a limited impact sand and gravel operation such as this, the annual report may be quite simple. Merely send us a note which briefly describes the progress of mining and reclamation over the last permit year and your plans for the next year. Please send us the fee and this brief report at your earliest convenience since both will be due as of your permit anniversary date of December 20, 1986. An examination of this file also shows me that you were to submit a form from the bank to properly assign your Certificate of Deposit No. 800755 to the State of Colorado. In this way, your mining operation would be properly bonded. Diana Knode sent this assignment form to you on September 5, 1986, and Regina of your office indicated to her on September 12, 1986 that the matter would be rectified as of September 15, 1986. To date, our file shows no response from you. I have enclosed another copy of the assignment form and Diana's cover letter. I strongly suggest that you respond to this simple problem immediately.
