In order to maintain your mining operation in compliance with the law, the annual
fees, report, and map required by C,R.S. 34-32-116(3) and (4) must be submitted
annually on or before your permit anniversary date. Fees are based upon a
schedule o i your operation is an open mining operation and 5275 for any
other operation. Division records indicate you owe 5350.00.

IF YOUR ANNUAL FEES, ANNUAL REPORT, AND MAP ARE NOT RECEIVED ON OR BEFORE YOUR
ANNIVERSARY DATE, A CIVIL PENALTY IN THE AMOUNT EQUAL TO YOUR ANNUAL FEE AMOUNT
WILL BE AUTOMATICALLY ASSESSED AND WILL BE DUE AND PAYABLE WITHIN THIRTY (30) DAYS
FROM YOUR ANNIVERSARY DATE.

this office as soon as possible, otherwise, we will expect to receive the annual
fee, the enclosed Notice of Intent to Continue Mining Operations (your annual
report), and your map, as required by the statute, on or before your anniversary
