Fees are based upon a <br />schedule of $350 if the operation is an open mining operation and $275 for any <br />other operation. Division records indicate you owe $275.00. <br />Permit No.: M-77-305 <br />Operation Name: CLP-21 Mine <br />Anniversary Date: 03/31/90 <br />Annual Fees for the Year(s): 1990 <br />Total: $275.00 (Due on the Anniversary Date) <br />IF THE ANNUAL FEES AND ANNUAL REPORT ARE NOT RECEIVED ON OR BEFORE THE <br />ANNIVERSARY DATE, A CIVIL PENALTY IN THE AMOUNT EQUAL TO-TRE~IAUICL FEE AMOUNT <br />WILL BE AUTOMATICALLY ASSESSED AND WILL BE DUE AND PAYABLE WITHIN THIRTY (30) <br />DAYS FROM THE ANNIVERSARY DATE. <br />If your records indicate different facts than those stated above, please inform <br />this office as soon as possible, otherwise, we will expect to receive the annual <br />fees as required by the statute on or before your anniversary date. Also, <br />please include the required Notice~o Tni;ent to Continue Mining Operations <br />(Annual Report to bring the Division up-to-date as to the progress of mining and <br />reclamation on the site) and the map. If you cannot include these materials with <br />the fees, please indicate on the enclosed sheet how soon we should expect to <br />receive the material. <br />Submittal of a report and map (to a scale of not less than 100' to the inch and <br />not more than 660' to the inch), indicating the phases of the reclamation plan <br />which have been completed and correlated with the timetable is required. The <br />report and map shall contain the following information: <br />1) Location and number of acres affected during that year; <br />2) locations and number of acres reclaimed for that year; <br />
