The fee is for last year even if the mining operation was in an inactive state. It is due and payable ON OR BEFORE the permit anniversary date shown below. BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A BOARD ORDER TO P CIVIL PENALTIES IN THE AMOUNT EQUAL TO THE ANNUAL FEE OF 5175.00 WILL BE AUTOMATICALLY ASSESSED.
