112 Annual Fee Letter PLEASE READ CAREFULLY
In order to maintain your mining operation in compliance with Colorado laws , the
annual fee , report, and map required by C.R.S. 34-32-116(3 and 4) and
34-32-127(2)(a)(II) (A-D) must be submitted annually on or before your permit
anniversary date. The annual fee (this is NOT a renewal fee) , report, and map are
for mining and reclamation during the PREVIOUS year , and are due and payable,
unless your reclamation bond has been released in its entirety, by your anniversary
date listed below.
Division records indicate you owe $490.00.

BOARD ORDER TO PAY CIVIL PENALTIES EQUAL TO YOUR ANNUAL FEE AMOUNT OF $490.00 WILL
AUTOMATICALLY BE ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL BE GRANTED
TO ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE
(C.R.S. 34-32-120) UNTIL THE VIOLATION IS ABATED.
If your records indicate different facts than those stated above or if you have
already forwarded your remittance , report, and map to us please inform this office
as soon as possible . Any questions concerning your annual report or fees , may be
directed to Barbara Chiappone at the Division office .
Please return the enclosed form with your completed Map , and a check or
money order for your annual fee , payable to the Mined Land Reclamation Division ,
