The Neglected Mine is operated under a Colorado Division of Minerals and <br />Geology (CDMG) 110(2) Limited Impact Operation (non-Designated Mining Operation) permit <br />for mining and operation of a froth flotation mill. The Permit Number is M-81-165, with an <br />anniversary date of October 2, 1999. <br />As required by C.R.S. 34-32-116(3)(a), please find enclosed MDI's completed annual report <br />form, a map, and a check for $225.
