fees, report, and map reouired by C.R.S. 34-32-116(3) and (4) must be submitted
annually on or before your permit anniversary date. Fees are based upon a
schedule o 35 if your operation is an open mining operation and E275 for any

To bring the Division up-to-date as to the reclamation on the site, please include
in your annual report, and map the following information:
Your map should be to a scale of not less than 100' to the inch and not more than
660' to the inch, it should indicate the phases of the reclamation plan which ha ve
been completed and correlated witn the timetable as reouired. Your report and map
shall contain the following information:
1) Location and number of acres affected during that year;
2) locations and number of acres reclaimed for that year:
