Jn September 18 1983, the Division of shined Land Reclamation
received an annual fee payment in the amount of ~ 50.00 . To this date,
we have no recd rd of haviny received the annual report as requested.
submission is required by the Colorado ~~tined Land Reclamation ,ict,
;article 3L of Title 34, C.R.S. 1973, as amended and by the Rules and
Regulations promulgated tnereunder. Your attention is directed to the
rollowing provisions from Rule L.4 for operators Holding a regular, 112
pemit and Rule 3.4 for operators holding a limited impact, 1)0 permit.
2.4 annual Reports
2.41 Un the anniversary date of each permit year, the operator snail
suo~nit a report and map (to a scale of not less tnan l0U' to Cne

Of the reclamation plan which have been CompleLed, correlated
with the timetaole. The report antl map Shall COntdin the
following information:
(1) vocation and numoer of acres affected during that year,
('L) Loca[ions and numoer of acres reclaimed for chat year,
(3) The location and numoer of acres in various stages of
reclamation (oackfilled, graded, topsoil replaced,
re vegetated),
(4) Tne species of vegetarian planted, the locations and the
approximate dates of planting,
(5) The type and approximate quantity of fertilizers, organic
;naterial or soil conditioners used and location of use,
(o) An appraisal of the success of reclamation efforts, and in
the event of failure of any efforts, a short narrative on
the suspected ~~r detennined causes of sucn failure,
(7) Estimates of the location and nwnoer of acres to oe
affected in the coming year, and
l~) Tne effective date of the infornation provided aoove snail
