Fees are based <br />upon a schedule of ;350.00 if the operation is an open mining operation and <br />5275.00 for any other operation. Division records indicate you owe E1050. <br />Permit No.: M-79-189 <br />Operation Name: SUPERIOR S 8 G PIT <br />Anniversary Date: O1 / 13 / 86 <br />1984, 1985, 1986 <br />Annual Fees for the Year(s): 51050 <br />Total: (Due on the Anniversary Date) <br />• <br />If your records indicate different facts, please inform this office as soon as <br />possible, otherwise, we will expect to receive the annual fees as required by <br />the statute on or before your anniversary date. Also, please include the <br />required Notice of Intent to Continue Plining Operations (Annual Report) to <br />bring the Division up-to-date as to the progress of mining and reclamation on <br />the site. If you cannot include these materials with the fees, please <br />incidate on the enclosed sheet how soon we should expect to receive the <br />material. <br />Submittal of a report and map (to a scale of not less than 100' to the inch <br />and not more than 660' to the inch), indicating the phases of the reclamation <br />plan which have been completed and correlated with the timetable is required. <br />The report and map shall contain the following information: <br />1) Location and number of acres affected during that year, <br />2) Locations and number of acres reclaimed far that year, <br />3) The location and number of acres in the various stages of <br />reclamation (backfilled, graded, topsoil replaced, revegetated), <br />4) The species of vegetation planted, the locations and approximate <br />dates of planting, <br /> <br /> <br /> <br />DEPARTMENT OF NATURAL RESOURCES <br />Davitl M.
