Cottony <br />Division Director <br />Natural Resource Trustee <br />Under the terms of your Reclamation Permit and Colorado Statutes, you must submit Annual <br />Fees and Annual Reports. The Division has received your annual fee and report. Please be <br />advised that due to the annual fee increase that went into effect July 1, 2007 you have a remaining <br />balance due. The Division is aware that you have complied with and sent the fee stated on your <br />annual fee and report request letter, but a balance of $ 103.00 is still needed. Please pay <br />remaining balance by close of business on August 31, 2007. The Division apologizes for the <br />inconvenience and thanks you for your understanding. <br />PLEASE NOTE: If the remaining annual fee balance is not received on or by August 31, 2007, <br />a formal notice of violation and a board order to pay civil penalties in the amount equal to the <br />annual fee will be automatically assessed. No permits for the new mining operations shall be <br />granted to any operator who is currently found to be in violation in this state (C.R.S. 34-32.5-120 <br />OR C.R.S. 34-32-120) until the violation is abated. <br />Division records indicate $103.00 fee is due.
