Of NATU:,AL ftESO:.".iCES <br />Based upon our records, the anniversary date of your mining operation permit is <br />t1arch and annual fees for the year(s) 79,80,81,82,83 totalling <br />S ,750.00 are due on that date. Fees are based upon a schedule of <br />5350.00 if the operation is an open mining operation of 10 acres or more and <br />5275.00 for any other operation 10 acres or more. <br />. Also, to bring t'~e Division up to date as to the mining and reclamation on the <br />site, please submit a report and map (to a scale of not less than 100' to the <br />inch and not more than 660' to the inch), indicating the phases of the reclama- <br />tion plan crhich have been completed, correlated with the timetable. The report <br />and map shall contain the following information: <br />1) Location and number of acres affected during that year, <br />2) Locations and number of acres reclaimed for that year, <br />3) The location and number of acres in the various stages of reclamation <br />fbackfilled, graded, topsoil replaced, revegetated), <br />4) The species of vegetation planted, the locations and approximate dates <br />of planting, <br />5) The type and approximate quantity of fertilizers, oceanic material or <br />soil conditioners used and location of use, <br />6) An appraisal of the success of reclamation efforts, and in the event <br />of failure of any efforts, a short narrative on the suspected or determined <br />causes of such failure, <br />7) Estimates of the location and number of acres to be affected in the <br />coming year, and - <br />8) The effective date of the information provided above shall be no more <br />than 45 days prior to the anniversary date. <br />C <br />
