(1989 Supp.) and Permit No. M-74-187 <br />require you to submit 5350.00 in annual fees and an annual report on or <br />before January 23 of each year. As of the date of this letter we fiave`not <br />rece ved your annual fees and report. Therefore, you are in violation of the <br />above cited statute and the provisions of your Permit No. M-79-187. Pursuant <br />to Section 34-32-124(7), C.R.S.
