MINED LAND RECLAMATION ANNUAL REPORT <br />Date: May 26, 2004 <br />Permit Number: M-2002-020 <br />Operation Name: 25`s Avenue Site <br />Pemuttee: City of Greeley <br />1, Extent of current disturbances. <br />Disturbances that occurred in 2003 aze related to the installation of inlet and outlet facilities For the lined <br />gravel lake facility and the mining of a 3-acre internal berm between the two existing gavel lakes on the <br />properly (see attached map). <br />2. Reclamation accomplished to date and during preceding year. <br />The reclaimed end use for the azea under the mining permit is water storage. The reclamation <br />accomplished to date revolved azound the construction of the slurry wall (completed in 2002) and inlet <br />and outlet facilities needed to operate the storage faciliTy..Irr 2003,.Greeley's 25's Ave. gravel lake storage <br />project met the Colorado State Engineer's Office performance standard for lined gravel pits. After <br />meeting this standard for lined reservoirs, Greeley inifiated and substantially completed the inlet and <br />outlet facilities for the storage facility, <br />3. New disturbances anticipated to occur during upcoming year. <br />The wet conditions in Colorado during 2003 allowed Greeley to contract with Lafarge, Inc. to begin <br />mining of a portion of the site rather than bringing the storage on-line to meet water demands. Greeley <br />plans to allow Lafazge to continue mining at the site for the next year. After the mining of the berm <br />between the two gravel lakes is complete, Lafarge plans to begin mining of the sand and gravel below the <br />bottom of the existing lakes (there is 15-20 ft of gravel remains between the bottom of the lakes and <br />bedrock). The maximum potential area of disturbance for 2004 is 62 acres as shown on the attached map. <br />4. Reclamation to be performed during upcoming year. <br />Finalizing the construction and testing of inlet and outlet facilities at the site. <br />5. Dates for the beginning of active operations. <br />Active mining began at the site in December of 2003. <br />6. Date active operation ceased for the year, if any. <br />Active operations have not ceased at the site. <br />
