Long <br />Urviaon Director <br />C.R.S. 34-32.5-116(3)(a) and (b), 1989 Supp. and Permit No. M-78-225L require you to submit to the <br />Division of Minerals and Geology $225.00 in annual fees and an annual report on or before April 28th of <br />each year. As of the date of this letter the Division has not received your annual fee and report for <br />April 28, 1995. Therefore, you are in potential violation of the above cited statute and the provisions of <br />your Permit No. M-78-225L. <br />If you fail to submit your annual fee and report to our office by October 18, 1995, this matter wilt be <br />heard at a Formal Public Hearing before the Mined Land Reclamation Board for consideration of permit <br />revocation at the October 19, 1995 Board Meeting. If you submit all past due annual fees and reports by <br />October 18, 1995, we will withdraw this matter. <br />IN ORDER TO AVOID PERMIT REVOCATION, AND POTENTIAL CIVIL PENALTIES, PLEASE SUBMIT <br />5225.00 FOR ANNUAL FEE(S) AND YOUR ANNUAL REPORT BY OCTOBER 18, 1995. <br />The Board Meeting will be held on October 19, 1995, in Room 318 of 1313 Sherman Street, Denver, <br />Colorado, beginning at 9:00 a.m. or as soon thereafter as the matter can be considered, and you will have <br />the opportunity to be represented and heard. <br />I( you have any questions, please contact H.
