In order to maintain your mining operation in compliance with the law, the
annual fees, report, and map required by C.R.S. 34-32-116(1)(a) mu t be
submitted annually on or before your permit anniversary date. Fee are based
upon a schedule of E350.00 if the operation is an open mining oper tion and
$275.00 for any other operation. Division records indicate you ow
E350.

possible, otherwise, we will expect to receive the annual fees as equired by
the statute on or before your anniversary date. Also, please incl de the
required Notice of Intent to Continue Mining Operations (Annual Re ort) to
bring the Division up-to-date as to the progress of mining and rec amation on
the site. If you cannot include these materials with the fees, pl ase
incidate on the enclosed sheet how soon we should expect to receiv the
material.
Submittal of a report and map (to a scale of not less than 100' to the inch
and not more than 660' to the inch), indicating the phases of the eclamation
plan which have been completed and correlated with the timetable i required.
The report and map shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and number of acres reclaimed for that year,
3) The location and number of acres in the various staves o
reclamation (backfiiled, graded, topsoil replaced, revegetated),
• 4) The species of vegetation planted, the locations and app oximate
dates of planting,
