Enclosed is the c mpleted new
form and an additional page regarding the operation. Last week I discussed this update /revised annual
report.
As the Division seems to be frequently altering annual report forms and procedures, it
up with what is acceptable and what is noc. In the original report form there was a con
which we stated there was no activity and none was anticipated. In that past, when tha
occurred, chat was always acceptable as there is nothing really to report and no map nt
submitted when nothing is happening. In this revision I have included an additional sh
addresses the condition of the operation and the issue of bonding, although the latter is
necessary as this is a Corporate Surety.
The additional sheet included with this revised annual report discusses the possibility
amendment being submitted in the next year. At this time it appears that a decision sh
regarding this prior to the next annual report becoming due. If chat is expected to pro
annual report will include a map showing the current permit and disturbance and the
be added to the permit as well as a brief discussion of the nature of the amendment.
If you have any additional questions, please call myself.
Respectfully,
~~ ~~
Mark A.
