However, the Division has not received a sufficiently completed annual report <br />and/or map. Failure to submit an annual report may result in enforcement action with associated civil <br />penalties. <br />Your attention is called to the requirements of C.R.S. 34-32.5-116 (3) (a): <br />Each year, on the arrniversnry date of the permit, an operntnr shnll submit the unrural fee <br />specified in section 34-32-127 (2), u report and map showing the extent of current dishrrbarrces <br />to affected land, reclanmtion nccomplished to date and during dte preceding year, new <br />distnrbunces dwt are anticipated to occrer during the upcoming year, reclmm~tion that will be <br />performed during the coming year, the dates for beginning of active operntiwts, and the date <br />attire operations ceased for dre year, if arty. <br />Enclosed is an annual report form with reporting instructions on the back. Please fill out the report <br />form and submit either a written report or an adequately labeled map as required by Rule 1.15, <br />and return it to the Division by November 1, 1999. <br />Thank you for you cooperation with this matter.
