Box 928 <br />Meeker, CO, 81641 <br />Subject: Submittal of Undesiraule Event Report: Re: White River Nahcolite <br />Dear Mr. Mehlhoff: <br />Enclosed please find Report of Undesirable Event regarding 2A-2V well head <br />leak on May 15, 1997. I called Mr. Paul Daggett of your staff 6-16-97 and <br />discussed this report 6-].7-97. A valve was left open on the annulus casing and <br />enough pressure built up in the cavity/casing, that pregnant liquor was forced out <br />the valve into a ditch and to the mud pit. It was fortunate that the drill pad had a <br />ditch and was sloped to the mud pit. The water (approx. 1900 gal.) was removed <br />from the mud pit and approx. 100 gal. soaked into the ground. <br />After the soil dries, surface contamination material will be disposed in the <br />approved lined pit. Reseeding will be not be not be necessary on the bare drill <br />pad. As the spill did not exit the mine site the CDH WQCD was not notified per <br />the Storm Water Dischzfrge Permit.
