Fees are based upon a <br />schedule of $350 if the operation is an open mining operation and $275 for any <br />other operation. If you cannot include these materials with <br />the fees, please indicate on the enclosed sheet how soon we should expect to <br />receive the material. <br />Submittal of a report and map (to a scale of not less than 100' to the inch and <br />not more than 660' to the inch), indicating the phases of the reclamation plan <br />which have been completed and correlated with the timetable is reouired. The <br />report and map shall contain the following information: <br /> 1) Location and number of acres affected during that year; <br />• 2) locations and number of acres reclaimed for that year; <br />
