Rule 2.04.13(1):By February 15, or other such date as agreed on, each permittee shall frle an
annual reclamation report covering the previous calendar year for all areas under bond. The report
shall include, but not be limited to, text, discussion and maps which address:
The report is in compliance with this rule. It was dated January 7, 2004 (cover letter), and received at
the Division on January 9, 2004. No map was included.
Ruie 2.04.13(1)(a): The name and address of the permittee and permit number;
The report is in compliance with this rule. The name and address of the permittee and permit
number are Minrec, Inc., 2768 Compass Drive, Suite 101, Grand Junction, Colorado 81506, Permit
Number C-81-025.
Rule 2.04.13(1)(b): Location and number of acres disturbed during that year;
The report is in compliance with this rule. No land was disturbed during 2003.
Rule 2.04.13(1)(c): Location and number of acres bac<~Iled and graded during that year;
The report is in compliance with this rule. No acres were backfilled and graded during 2003.
Rule 2.04.13(1)(d): Location and number acres topsoiled during that year;
The report is in compliance with this rule. No land was topsoiled during 2003.
Rule 2.04.13(1)(e): The species, location and number of acres of vegetation planted during that
year, including any augmented seeding or cultural practices; and
0.2 acres were permanently seeded during 2003. No Weed control was performed.
