Fees are based upon a <br />schedule of $350.00 if the operation is an open mining operation and $275.00 for <br />any other operation. Also, please include the required <br />Notice of Intent to Continue Mining Operations (Annual Report) to bring the <br />Division up-to-date as to the progress of mining and reclamation on the site. If <br />you cannot include these materials with the fees, please incidate on the enclosed <br />sheet how soon we should expect to receive the material. <br />Submittal of a report and map (to a scale of not less than 100' to the inch and <br />not more than 660' to the inch), indicating the phases of the reclamation plan <br />which have been completed and correlated with the timetable is required. The <br />report and map shall contain the following information: <br />1) Location and number of acres affected during that year, <br />2) Locations and number of acres reclaimed for that year, <br />3) The location and number of acres in the various stages of reclamation <br />(backfilled, graded, topsoil replaced, revegetated), <br />4) The species of vegetation planted, the locations and approximate dates <br />of planting, <br />215 Centennial Building, 1313 Sherman Street Denver, Colorado 80203-2273 Tel.
