In order to maintain your mining operation in compliance with the law, the annual fees, report, and map required by C.R.S. 34-32-116(3) and (4) must be submitted annually on or before your permit anniversary date. fees are based upon a schedule o i your operation is an open mining operation and 5275 for any other operation. If your records indicate different facts than those stated above, please inform this office as soon as possible, otherwise, we will expect to receive your annual fee, the enclosed Notice of Intent to Continue Mining Operations (your annual report), and your map, as required by the statute, on or before January 4, 1990. If you cannot include these materials with the fee, p ease in icate on the enclosed annual report form how soon we should expect to receive these materials. To bring the Division up-to-date as to the reclamation on the site, please include in your annual report, and map the following information: Your map should be to a scale of not less than 100' to the inch and not more than 660' to the inch, it should indicate the phases of the reclamation plan which have been completed and correlated with the timetable as required. Your report and map shall contain the following information: . 1) Location and number of acres affected during that year; 2) locations and number of acres reclaimed for that year;
