As a response to my courtesy call to your office yesterday, made as a reminder that we have reached the decision
date without adequately addressing the requirements of the 110 application you filed, you have subnutted the third
request for an extension for the above-named application. This request asks for an extension of the decision date
until July 1, 1999. I am approving this request, and the decision date is now set for 7/1/99.
As you are aware, if the requirements are not met by the decision date, the application may be denied. Therefore,
please ensure that all responses to the Division's 11/18/98 adrequacy review letter and onsite inspection report of
l/13/99 aze submitted with enough time for them to be reviewed, and if necessary request further clarification from
you. If the new date cannot be met, you may consider requesting another extension.
As you requested in your letter, I will call you regarding this extension approval.
