The fee is for last year even if the mining operation was in an inactive state. It is due and payable ON OR BEFORE the permit anniversary date shown below. PLEASE NOTE: IF THE ANNUAL FEE, ANNUAL REPORT, AND MAP ARE NOT RECEIVED ON OR BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A BOARD ORDER TO PA CIVIL PENALTIES IN THE AMOUNT EQUAL TO THE ANNUAL FEE OF $175.00 WILL BE AUTOMATICALLY ASSESSED.
