Rule 4.0~.13(4)(c): A hydrologic report shall be submitted to the Division annually with the dote
ojsubmittal determined in consadtntion +vith the permittee. The ann:~al hydrologic report shat!

The report was received in the Division of Minerals and Geology (Division) on February 10. 1999
under a Stephen Self letter of January 28, 1999. No submittal date is stated in the permit ("An
annual hydrology report will be submitted to the Division afrer completion of the calendar year."
This might be a mid-term review item.).
Rule 4.0~.14(4)(e)(i): Water yunntity darn obtained from each monitoring site during the venter
year;
